Atomically Apply the reviewed plan as the actual user, publishing main only on success. The stack's rules must be met (REVIEW_REQUIRED, CHANGES_REQUESTED) unless a stack admin bypasses them.
Arguments

planIdIaCPlanID!Server-produced plan reviewed by this user.
digestString!Exact digest approved by this user; stale plans fail closed.
bypassReasonStringApply without meeting the stack's rules, recording this reason (at most 2 KiB). Only stack admins, and only when the rules allow bypass (otherwise BYPASS_NOT_ALLOWED). A blank reason is no bypass.
